Your first visit to Revive Neuromuscular Aesthetic Dentistry establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. As with any new relationship, getting to know one another is the first step in determining whether or not our office will be a good ‘fit’ for you. It’s a great way to explore a new world of possibilities for your smile. Dr. Yazdani & Dr. Mortazie like to see the new patients first before scheduling any cleanings or other treatments.
We will send you The New Patient Registration Forms to fill out through e-mail and/or text. For your convenience, all these forms are electronic, so when you are completed, they will immediately go into your patient chart with our office. Please have these forms completed if possible two days before your appointment, so that we have time to review them, and tailor your visit to your wants and needs, as well as expedite your visit.
What To Expect From Your First Visit
On your first visit you will find out what sets us apart from other dental offices. Getting started with Dr.’s Yazdani & Dr. Mortazie is easy. When you arrive, our New Patient Care Coordinator will meet with you. The two of you will sit down, she will listen to you and gather all necessary information the Doctor needs to help with his assessment. This will include a set of digital photographs made of your mouth and dental x-rays. Shortly after one of the Doctors will see you for a comprehensive examination of your gums and teeth. You and the doctor will then be able to collectively discuss your concerns. Your first examination with us will be for an hour of our undivided attention. It is very important that the you play an active role in your treatment selections and that you are fully aware of all of your options. We look forward to having you as a lifelong patient!
